ABSTRACT:Investigation of accidents involving fire and explosion is essential for learning the root causes and for implementing improved safety measures. A thorough analysis often gives rise to a number of unknowns in the causal events requiring additional research. This webinar will re-visit major accidents, such as Piper Alpha and Buncefield, to illustrate how follow-up research enables understanding of why poor design, operation and maintenance can lead to fires and explosions which then escalate into more serious events. It will be shown that a necessary feature of a safety assessment requires a full systems analysis to identify, assess and avoid these tragic events.
